Job Overview
We are looking for a proactive and organised Care Coordinator Administrator to support the delivery of the Enhanced Health in Care Homes (EHCH) framework. This role is ideal for someone who enjoys working collaboratively, communicating clearly, and helping improve care for residents in local care homes.
Hours:
14 hours per week, Tuesday to Friday, 09:00-12:30.
Main Responsibilities
- Provide administrative and operational support to the Care Home Nurse Lead and wider PCN team.
- Monitor and manage the care home email inbox, ensuring timely responses.
- Coordinate MDT meetings and routine care home visits, including preparation and follow‑up.
- Maintain accurate patient registers and systems to support EHCH compliance.
- Process hospital discharge communications and liaise with community teams to ensure actions are completed.
- Support shared decision‑making initiatives, including RESPECT and personalised care plans.
- Assist with vaccinations, annual reviews, and scheduling of routine care.
- Organise and attend regular care home meetings, documenting actions and ensuring consistent communication.
- Carry out additional administrative duties as required.
Essential Requirements
- Experience working in a care home or care‑related setting
- Full UK driving licence (travel to care homes required)
- Strong communication and IT skills (Microsoft Office & Outlook)
- Ability to work independently and as part of a team
- Flexible, organised, and confident working in a changing environment
Desirable
- Experience in primary care or NHS systems (EMIS/SystmOne)
- NVQ Level 2 or equivalent administrative experience
