Farnham Integrated Care Services is working as an extension of Farnham GP Practices to provide an acute same day service (SDS) in an independent setting.
The ideal GP candidate will work autonomously as part of the SDS team, providing high quality medical care to the Farnham PCN population.
Salary is £11,100 - £11,300 per session per annum. With up to 7 sessions available per week.
A September start date is preferred. Interviews will be scheduled week commencing 17th August.
The role requires the delivery of excellent acute clinical care whilst adhering to General Medical Council (GMC) standards and contractual obligations, fostering positive collaboration within the multidisciplinary team at FICS and the Farnham PCN Practices.
This role will not case manage ongoing patient care, review test results or provide follow ups.

The main purpose of our PCN is to enhance patient outcomes, improve the quality of care and increase the efficiency of healthcare delivery. As a PCN we achieve these goals through improved coordination of care, offering a wide range of services, enhancing access to care, managing population health, promoting preventive care, health education and prioritising a patient-centred approach.
Our ultimate aim is to deliver better health outcomes for the local population, as well as a reduction in health inequalities. We work alongside Farnham Integrated care services which houses some of our clinicians, the same day service and enhanced access.
Farnham Integrated Care Service is a separate entity which is the vehicle for implementing locality plans and supporting high quality integrated care in Farnham.
Primary Responsibilities
Clinical Care
- Deliver high-quality medical care to the PCN entitled population, with a focus on acute same-day care and integrated services.
- Prescribing in line with local and national guidelines.
- Conduct telephone triage for patient assessments and care recommendations.
- Maintain accurate and compliant clinical records, following best practice and guidance.
- Administrative Responsibilities
- Process referrals and other related administrative tasks promptly.
- Perform regular quality checks of referrals and feedback.
- Team Collaboration and Development
- Work collaboratively within the MDT, sharing clinic workload equally and promoting a positive, cooperative work environment.
- Support the training and mentoring of various clinical roles.
- Attend and contribute to team meetings, actively supporting the development and continuous improvement of clinical governance and service performance.
- Quality Improvement and Service Development
- Lead and participate in quality improvement initiatives, ensuring the service adheres to clinical guidelines and follows best practices through audits and feedback.
- Encourage a culture of continuous improvement, collaborating with all staff and liaising regularly to enhance service efficiency.
- Engage in strategic planning with directors, aiming to improve clinical performance and resource utilisation.
- Support clinic coordination to ensure effective resource use, delivering high-quality service.
- Your principal place of work will be within Farnham Integrated Care Services. But under extenuating circumstances you may also be required to undertake visits to housebound patients where absolutely necessary to preserve business continuity and clinical duty.
Person Specification.
Essential
GMC registered with a licence to practise in the UK
On the GMC GP Register and included on the NHS England Performers List.
MRCGP qualified or equivalent.
Experience working as a GP in a primary care setting.
Right to work in the UK.
Subject to Disclosure and Barring Service Check (formerly known as CRB) to check for any previous criminal convictions.
