The Parent and Carer Transitions Worker plays a unique role in the lives of parents and carers. They will provide support to families of young people aged 14+ with additional support needs as they navigate the transition to adulthood. The primary focus is on prevention, capacity building, and enhancing community and social connectedness. By intervening early and strengthening family capability, the service aims to reduce the likelihood of crisis escalating and promote sustainable independence outcomes for the young person. Engagement with the Parent & Carer Transitions Worker will help families develop greater confidence in supporting their young person’s journey towards independence, alongside an improved understanding of adult services, community resources, and available pathways. They will experience reduced anxiety and isolation, and increased resilience and day‑to‑day capacity, enabling them to provide more effective and sustained support.
We encourage staff to bring their own individual skills and style to the role of Parent and Carer Transitions Worker and recognise that people will carry out their role in very different styles, which is something that we embrace. We expect The Parent and Carer Transitions Worker to use their judgement and common sense while supporting families. Support and advice will be available from a manager as required.
The Parent and Carer Transitions Worker is required to carry out their work flexibly to meet the needs of the parents and carers. We actively support and encourage colleagues to have at least 2 days per week when they are not engaged in Neighbourhood Networks business. In some instances, the Parent and Cares Transitions Worker may have to work at evenings and weekends depending on the needs of the families.
Other aspects of the role may include: Taking part in own supervision, liaison and networking in the local neighbourhood/community, Neighbourhood Network meetings, training and events, travel time, phone calls, administration work for weekly monitoring forms, processing expenses, etc. If multiple issues arise simultaneously, the Parent and Carer Transitions Workers will need to use their judgement to prioritise their workload, which can be done in consultation with their Manager.
