Job Summary
We are seeking a dedicated and compassionate Care Co-ordinator to oversee and organise care services within a senior care environment. The successful candidate will play a vital role in ensuring high-quality care delivery, supervising staff, and maintaining effective communication with residents, families, and healthcare professionals. Responsibilities
- Organise and oversee daily care routines for residents in the care home setting
- Develop, implement, and review personalised care plans in collaboration with residents and their families
- Supervise and support care staff to ensure high standards of care are maintained
- Coordinate with healthcare professionals to facilitate medical appointments and treatments
- Maintain accurate documentation related to resident care and progress
- Lead on training initiatives to enhance team skills and ensure compliance with care standards
- Communicate effectively with residents, families, and external agencies to address needs and concerns
- Ensure adherence to health and safety regulations within the care environment
- Manage scheduling and resource allocation efficiently to optimise service delivery
Experience
- Proven experience working within a care home or senior care setting
- Supervising or leadership experience within a healthcare or social care environment
- Strong driving skills with a valid driving licence preferred for travel between sites or appointments
- Familiarity with creating and managing care plans tailored to individual needs
- Excellent communication skills, both verbal and written, with the ability to liaise effectively with diverse stakeholders
- Proficient in IT systems relevant to healthcare documentation and communication tools
- Knowledge of regulatory requirements related to adult social care services

Pay: £14.00-£16.00 per hour
