Care Coordinator & Assessor

Home Sweet Home Care is seeking an experienced and dedicated Care Coordinator & Assessor to join our growing team. To coordinate and oversee individualized care plans for clients, ensuring high-quality,person-centred support that meets health, social and wellbeing needs. The coordinator acts as the central point of contact between clients, families, health professional and service providers to ensure safe, effective and integrated care id delivered.
Reporting to Care Manager/Lead Care Assessor
Essential Requirements
- Full UK driving licence and access to your own vehicle.
- Previous experience in domiciliary care.
- Previous experience as a Care Coordinator.
- Good understanding of the homecare sector.
- Excellent communication, organisational, and problem-solving skills.
Desirable Requirements
- Previous experience or knowledge of complex care services.
Key Responsibilities Client Assessments and Care Planning
- Promote the services provided by Home Sweet Home Care.
- Conduct initial assessments of care needs for prospective clients.
- Complete associated risk assessments.in partnership with service users and
- their families
- Develop person-centred care plans that accurately reflect clients' needs, goals, preferences, and aspirations.
- Review and update care plans as required.
- Coordinate multiple runs and areas
- Build and maintain positive relationships with care staff.
- Ensure staff supervision, competency assessments, and support visits are completed and remain up to date.
- Care Coordination
- Manage and monitor staff schedules using rostering software to ensure seamless care delivery.
- Reallocate visits and shifts effectively to cover absences and service demands.
- Monitor attendance and punctuality using electronic monitoring systems.
- Ensure staffing levels are maintained to meet client requirements.Communication and Partnership Working
- Liaise with families, social services, healthcare professionals, and other stakeholders to ensure care provision remains appropriate and effective.
- Report significant changes in service users' health, wellbeing, or support needs to the relevant manager.
- Maintain accurate records using company systems and processes.Compliance and Quality Assurance
- Adhere to company policies, procedures, and regulatory requirements.
- Promote confidentiality, safeguarding, equality, diversity, and inclusion.
- Participate in relevant training and professional development activities.

- Schedule Monday to Sunday on a rota basis 5 days on 3 days off. Office Based with community Visits. May require travel within the local area
- Hours to be covered are 5am to 2pm or 2pm to 11pm
Pay: £32,000.00 per year
