To provide compassionate, person-centred support to individuals living in supported accommodation. The role involves helping people maintain their independence, exercise choice and control, develop daily living skills, and participate fully in their community.
Support must be delivered with dignity and respect and in accordance with each person’s support plan, risk assessments, preferences and agreed outcomes.
Key Responsibilities
- Provide individualised support based on each person’s needs, abilities, goals and preferences.
- Encourage individuals to make informed choices and take an active role in decisions about their lives.
- Support people with everyday activities, including cooking, cleaning, shopping, budgeting and maintaining their homes.
- Provide personal care sensitively and respectfully where this is included in the person’s support plan.
- Support individuals to attend appointments, education, employment, social activities and community events.
- Assist with medication in accordance with organisational procedures, agreed support plans and relevant training.
- Promote independence by supporting individuals to develop and maintain practical and social skills.
- Build positive, professional relationships while maintaining appropriate boundaries.
- Recognise and respond appropriately to changes in an individual’s health, behaviour or emotional wellbeing.
- Follow support plans, positive behaviour support strategies, risk assessments and healthcare guidance.
- Maintain accurate, clear and confidential records, including daily notes, incident reports and medication documentation.
- Communicate effectively with individuals, relatives, colleagues, healthcare professionals and other agencies.
- Safeguard individuals from abuse, neglect, exploitation and avoidable harm, reporting concerns immediately.
- Respond calmly and appropriately to emergencies, incidents and challenging situations.

- Attend staff meetings, supervision sessions and mandatory training.

- A relevant qualification in health and social care.

- Experience providing personal care or supporting medication.
- Knowledge of person-centred practice, safeguarding and positive risk-taking.
- A full driving licence and access to a suitable vehicle where required by the service.

The successful candidate will be required to complete appropriate pre-employment checks, including references, proof of the right to work and a satisfactory enhanced DBS check where applicable.
