Summary:
The Care Coordinator will be responsible for coordinating clients' care services, advocating for their needs, and ensuring effective communication between healthcare professionals, client, and their families or caregivers.
Responsibilities:

Coordinate and manage client care services based on individual needs and resources
- Conduct comprehensive assessments and develop individual care plans
- Collaborate with healthcare professionals and other care coordinators
- Advocate for clients' access to necessary health services and resources
- Schedule and monitor appointments, referrals, and care transitions
- Communicate with clients, carers, and families to provide guidance, management, and care strategies
- Maintain accurate client records and care plans
- Participate in team meetings and quality improvement initiatives
- Coordinate the overall care team
- Other duties as assigned.
Qualifications:

NVQ 3 - 5 or working towards
- Minimum of two years of experience in a senior healthcare or care coordination position
- Knowledge of healthcare delivery systems and care coordination techniques
- Excellent organizational, communication, and interpersonal skills
- Ability to work independently or as part of a team
- Strong analytical and problem-solving skills
- Commitment to person-centered care
- Proficiency in electronic health records and Microsoft Office Suite
