Occupational Therapist – Right Size Care Team
Are you an enthusiastic Occupational Therapist with a passion for promoting independence and improving outcomes for people receiving care and support?
Following successful pilot project, we have had significant investment in our Occupational Therapy workforce and are looking for motivated Occupational Therapists to join our innovative Right-size Care Team. The team has recently been nominated for Social Care Team of the Year at the National Back Exchange Excellence Awards. This exciting role focuses on ensuring people receive the right support, at the right time, in the right way by promoting strength-based practice, proportionate care, rehabilitation, enablement and the effective use of equipment and technology.
Working collaboratively with social workers, care providers, health professionals and families, you will review existing care packages, identify opportunities to maximise independence and reduce unnecessary reliance on formal care where appropriate, while ensuring safety, dignity and person-centred outcomes remain at the heart of every decision.
You will also play a key role in developing the knowledge and confidence of care providers through education, coaching and promoting best practice in moving and handling and Right Size Care principles.
What You’ll Be Doing
- Undertaking holistic Occupational Therapy assessments.
- Reviewing existing care packages using a strengths-based approach.
- Identifying opportunities to promote independence through rehabilitation, equipment and assistive technology.
- Supporting proportionate care and Right Size Care initiatives.
- Working alongside care providers to develop innovative care solutions.
- Providing advice and training relating to moving and handling and safer care delivery.
- Building positive relationships across health and social care partners.
- Monitoring outcomes and demonstrating the impact of Right Size Care.
- Supporting service development and continuous improvement.
About You
You will be:
- A qualified Occupational Therapist registered with the HCPC.
- Passionate about promoting independence and enabling people to achieve their goals.
- An excellent communicator with strong presentation and influencing skills.
- Confident challenging traditional models of care using evidence-based practice.
- Experienced in multidisciplinary working.
- Able to demonstrate sound clinical reasoning and risk management.
- Committed to person-centred, strengths-based practice.
- Flexible, organised and able to manage a varied caseload.
Desirable experience includes:
- Adult Social Care.
- Reablement or rehabilitation services.
- Moving and Handling.
- Assistive Technology.
- Care package reviews.
- Service improvement projects.
