We have an opportunity for an innovative, skilled health professional or social worker to join our team who provide a commissioning service to adults and children with a learning disability and/or autism at a time when they need support from mental health services. This can be around preventing hospital admission and discharge planning, including commissioning community-based services to meet individual needs.
Provide case management support to autistic people and people with a learning disability who are at risk of hospital admission, or supporting discharge for these people who are currently receiving inpatient treatment., including identifying or commissioning services to meet individual need.
Inputting into the referral and review processes for the Dynamic Support Register.
Undertaking Care (Education) and Treatment Reviews

You will receive high quality supervision, support and training to further develop your skills and experience to support autistic people and people with a learning disability.
The service operates Monday to Friday, from 9am to 5pm. Although, we understand the importance of work/life balance and would welcome discussions around flexible working arrangements as required.
To provide case management to people under the LDA Programme.
Manage own delegated portfolio across the LDA Commissioning team.
To provide management and leadership to support staff working in the LDA Commissioning Team to deliver on the he LDA Programme.
To work with people with a learning disability and/ or autism to maximise their independence with the appropriate support and services.
To plan, promote and commission services.
To ensure the service delivery both directly and commissioned are in line with national policy and guidance in relation to the Dynamic Support Register (DSR), care, education and treatment reviews (C(E)TRs) and Case Management in a person centred and individualised approach.
