Overview
We are seeking a dedicated and compassionate Health Care Professional to join our team. This role involves providing high-quality care and support to individuals in various settings, including care homes and home care environments. The successful candidate will be responsible for assisting with daily activities, developing personalised care plans, and ensuring the wellbeing of clients. Responsibilities
- Deliver personalised care and support to clients in care homes or home care settings, ensuring their comfort and dignity are maintained at all times.
- Assist clients with daily activities such as mobility, personal hygiene, medication management, and meal preparation.
- Develop, implement, and review individualised care plans in collaboration with healthcare teams and family members.
- Maintain accurate documentation of care provided, adhering to organisational policies and legal requirements.
- Communicate effectively with clients, families, healthcare professionals, and team members to coordinate care and address any concerns promptly.
- Utilise IT systems to record client information, update act , and manage schedules efficiently.
- Drive or accompany clients as needed for appointments or outings, ensuring safe transportation and adherence to safety protocols.
- Uphold health and safety standards within all aspects of work to ensure a secure environment for clients and staff.
Requirements
- Proven experience working within a care home or home care environment is desirable but not essential; training will be provided.
- Valid driving licence is preferred to facilitate transportation responsibilities or ability to commute if you will be allocated to working in care homes
- Excellent communication skills in English, both written and verbal, to interact effectively with clients and colleagues.
- Strong IT skills for managing electronic records and care planning software.
- Ability to work flexibly across various shifts as required by the organisation’s operational hours.
- Compassionate nature with a genuine interest in providing high-quality person-centred care.
- Good organisational skills with the ability to prioritise tasks efficiently.
- Ability to work independently whilst maintaining effective teamwork when necessary.
