Are you an experienced care professional with a passion for improving outcomes for older people?
Lincolnshire Care Association (LinCA) is looking for a motivated and knowledgeable Care Home Trusted Assessor to join our established team. This rewarding role offers the opportunity to work directly with health and social care colleagues, supporting safe, timely and effective discharge from hospital into residential and nursing care settings.
As part of the National Care Home Trusted Assessor Programme, you'll play a vital role in ensuring individuals are assessed appropriately and can move into the right care setting without necessary delays.
About the Role
Working alongside the integrated health and social care team, you will support residents who are either living in, or require admission to, a residential or nursing home following a stay in hospital.
You will:
- Complete care home assessments to support safe and effective discharge planning.
- Working closely with hospital teams, care providers, residents and their families.
- Liaise with local residential and nursing homes to identify suitable placements.
- Visit care homes as required to support assessment and decision-making.
- Help reduce delayed discharges and improve the experience of individuals moving between services.
- Provide professional advice and support to colleagues across the health and social care system.
About You
You will be passionate about delivering high-quality care and have a strong understanding of the adult care sector.
Essential Requirements
- Minimum of 5 years' experience in health and social care.
- Significant experience working within elderly residential and/ or nursing care.
- Excellent communication and relationship-building skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong assessment, problem-solving and organisational skills.
- Full UK driving licence, and access to your own vehicle, as travel between locations is an essential part of the role.
Desirable Experience
- Registered Nurse (PIN not essential)
- Care Home Manager, including nursing services.
- Deputy Care Home Manager.
- Extensive experience within adult social care services.
