We are looking for caring, motivated and enthusiastic individuals to join our Reablement Team as Support Workers.
Our service supports adults to regain independence, rebuild confidence and achieve their personal goals following illness, injury, hospital discharge or a change in circumstances. Working in people's homes and local communities, you will provide short-term support that enables individuals to do as much as possible for themselves rather than doing tasks for them.

Key Responsibilities
Support individuals to achieve goals identified within their reablement plan.
Assist people to develop and regain daily living skills, including personal care, meal preparation and mobility.
Encourage independence and promote choice, dignity and wellbeing.
Support service users following discharge from hospital or periods of ill health.
Work collaboratively with Occupational Therapists, Social Workers, healthcare professionals and family members.
Maintain accurate records and provide feedback on progress and outcomes.
Follow safeguarding procedures and health and safety requirements at all times.
Build positive and trusting relationships with service users and carers.
About You -

Can work independently and as part of a team.
Is organised and able to maintain accurate records.
Has a flexible approach to work.Experience in care, support work, health or social care is desirable but not essential, as full training will be provided.
