We are seeking a passionate and Experienced Social Worker to join our Community Led Support Team. This is an exciting opportunity to work with adults aged 18 and over, with identified care and support needs, supporting them to live as independently as possible while promoting wellbeing, choice, control and safeguarding.
This role offers the opportunity to build meaningful relationships with people, advocate for their rights, and make a real difference every day.
As an Experienced Social Worker, you will hold a challenging, varied yet rewarding caseload, undertaking strengths-based assessments, support planning, safeguarding enquiries, reviews, and complex casework. You will work closely with individuals, families, carers, health professionals, and partner agencies to deliver person-centred outcomes.
You will use your professional expertise to balance risk, promote positive outcomes, and ensure that adults receive high-quality support tailored to their individual circumstances.
We are looking for a confident and knowledgeable practitioner who is committed to delivering excellent social work practice.
To be successful, you will have:
- Degree or postgraduate qualification in Social Work.
- Registration with Social Work England.
- Significant post-qualification experience within adult social care.
- Strong knowledge of relevant legislation including:
- Care Act 2014
- Mental Capacity Act 2005
- Human Rights Act 1998
- Safeguarding Adults legislation and guidance
- Experience of undertaking complex assessments and risk management.
- Excellent communication and relationship-building skills.
- Ability to work effectively within a multi-disciplinary environment.
- Strong analytical and decision-making abilities.
- A commitment to person-centred, strengths-based practice.
You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).
The ability to travel throughout the Borough will be required and access to a car to undertake visits. This may include travel outside of the Borough of Swindon.
The team operates a hybrid working model with all practitioners expected to be in the office for a minimum of two days per week.
You will be subject to an enhanced Disclosure and Barring Service Check.
