Job Overview
We are seeking a dedicated and organised Care Co-ordinator to oversee and facilitate high-quality care services within a senior care environment. The successful candidate will play a vital role in ensuring the well-being of residents, coordinating care plans, and supervising staff to deliver compassionate support. Responsibilities
- Develop, implement, and review personalised care plans for residents, ensuring individual needs are met effectively.
- Coordinate daily care activities, ensuring smooth communication between staff, residents, and families.
- Supervise and support care staff, providing guidance and leadership to maintain high standards of care.
- Organise staff rotas and allocate resources efficiently to meet service requirements.
- Maintain accurate documentation of care activities, incidents, and resident progress using IT systems.
- Liaise with healthcare professionals, families, and external agencies to coordinate additional support or services as required.
- Ensure compliance with health and safety regulations and promote best practices within the care environment.
- Support new staff through training and mentoring programmes to foster professional development.
Skills
- Proven experience in supervising or managing within a care home or senior care setting.
- Strong leadership qualities with excellent communication skills to liaise effectively with residents, families, and team members.
- Competent IT skills for managing care plans, documentation, and communication tools.
- Valid driving licence is desirable for facilitating transportation needs or external appointments.
- Knowledge of care planning processes and regulatory standards within senior care environments.
- Organised with the ability to prioritise tasks efficiently while maintaining attention to detail.
- Compassionate approach with a genuine commitment to enhancing the quality of life for older adults.
Pay: £24,420.00-£42,879.78 per year
