We’re looking for a person-centred and proactive individual to join our Active North Yorkshire team as an Active Health Practitioner. This role focuses on enabling people to make sustainable lifestyle changes that improve their physical and mental wellbeing.
About the Role:
As part of the Active North Yorkshire team, you’ll deliver inclusive and tailored health and wellbeing programmes that promote physical activity, healthy habits, and positive behaviour change. Whether you’re leading in-person sessions, delivering virtual workshops, or working one-to-one with clients, you’ll help individuals build confidence, motivation, and resilience to take control of their health.
Key Responsibilities:
Deliver lifestyle-focused interventions – Conduct health assessments, co-create personalised wellbeing plans, and facilitate sessions that support long-term behaviour change.
Promote physical activity and healthy choices – Encourage participants to integrate movement and wellbeing practices into their daily lives.
Engage diverse communities – Work with individuals from a range of backgrounds, including those in rural areas, younger adults, and people with long-term health conditions.
Collaborate with partners – Build strong relationships with local organisations, healthcare professionals, and community groups to widen access and participation.
Support sustainable change – Provide ongoing encouragement, track progress, and celebrate achievements to help individuals maintain their wellbeing goals.
What We’re Looking For:
Experience in delivering health and wellbeing programmes, ideally with a focus on physical activity, lifestyle coaching, or behaviour change.
Excellent communication and interpersonal skills, with the ability to inspire and engage individuals and groups.
Knowledge of behaviour change models and a holistic, person-centred approach to health improvement.
Strong organisational and IT skills, with a proactive and supportive attitude.
