NR Care is a well-established domiciliary care provider delivering high-quality, person-centred care across Norfolk. We are looking for an experienced and motivated Care Coordinator to join our growing team and play a key role in the day-to-day management of our care service.
This is a varied role combining care coordination, rota management, client assessments, care planning, staff support and on-call responsibilities. The successful candidate will work closely with the Registered Manager to ensure the safe, effective and compliant delivery of care services.
Key Responsibilities
- Create and manage staff rotas to ensure all care visits are covered safely and efficiently.
- Respond proactively to staffing changes, sickness and emergencies.
- Conduct new client assessments and complete person-centred care plans.
- Carry out care plan reviews and risk assessments.
- Liaise with clients, family members, healthcare professionals and local authorities.
- Support the recruitment, induction and onboarding of new care staff.
- Conduct spot checks, competency observations and service reviews.
- Maintain accurate records and ensure compliance with CQC requirements.
- Support the Registered Manager with quality assurance and service improvement activities.
- Cover care calls when required to support service delivery.
- Participate in the out-of-hours on-call rota.
About YouEssential
- Minimum 2 years' experience in a Care Coordinator, Senior Carer or similar role.
- Experience creating and managing staff rotas.
- Experience completing care plans, risk assessments and reviews.
- Strong understanding of CQC regulations and domiciliary care standards.
- Excellent communication and organisational skills.
- Good IT and administration skills.
- Full UK Driving Licence and access to a vehicle.
- Ability to participate in the on-call rota.
Desirable
- Level 3 Diploma in Health & Social Care (or equivalent).
- Previous domiciliary care experience.
- Experience using electronic rostering and care management systems.
- Experience undertaking spot checks and staff supervision.

Pay: £28,000.00-£35,000.00 per year
