Job Summary
We are seeking a compassionate and dedicated Practitioner to provide high-quality care within a supported accommodation setting for young people aged 16-18.

The successful candidate will play a vital role in supporting young people' wellbeing, ensuring their comfort, safety, and dignity are maintained at all times. Duties
- Deliver personalised support plans tailored to individual needs, promoting independence and well-being
- Assist young people with daily activities such as meal preparation, life skills, and emotional support.
- Monitor young people’s health and report any changes or concerns to senior staff promptly
- Maintain accurate documentation of care provided using IT systems and ensure records are up-to-date
- Support young people with medication ordering and storage following prescribed guidelines
- Foster positive communication with young people, their families, and team members to ensure holistic care delivery
- Assist with organising activities that promote social engagement and mental stimulation
- Ensure the safety and cleanliness of the home environment in accordance with health and safety regulations
Experience
- Previous experience working in a supported accommodation/ working with young people care is desirable but not essential; full training will be provided for suitable candidates
- Good understanding of English, both spoken and written, is essential for effective communication with residents and team members
- Basic IT skills are required for maintaining electronic care records and documentation
- Excellent communication skills are vital to build trust and rapport with young people, families, and colleagues.
