What You’ll be doing
Join a dynamic multi-disciplinary reablement team dedicated to supporting individuals in maintaining their independence within the community. In this role, you will contribute to a collaborative approach that focuses on empowering people to regain confidence and practical skills needed for daily living.
You will help deliver a responsive, customer-led service operating seven days a week, ensuring individuals receive the right support at the right time. The role centres on promoting, achieving, and sustaining independence, enabling people to remain safely and comfortably in their own homes and communities.
What we’re looking for
We're looking for a caring and motivated Reablement Support Worker to help people regain their independence and continue living safely in their own homes. You'll support individuals with personalised care and therapeutic activities, encouraging them to build confidence and carry out everyday tasks independently. You'll communicate effectively with service users, families, carers, and colleagues, use IT systems to maintain accurate records, and identify and report any concerns when needed. A good understanding of equality, diversity, inclusion, and reablement approaches is essential.
We're looking for someone who is proactive, flexible, and able to work using their own initiative. You'll need to be comfortable with manual handling and working a rota that includes evenings, weekends, and some bank holidays. A full UK driving licence and access to a vehicle are essential, as you'll travel across the borough each day. You'll also need an NVQ Level 3 in Health and Social Care (or equivalent), or be willing to achieve this within 12 months of joining.

Because of the nature of this job, it will be necessary for the appropriate level of criminal record disclosure to be undertaken.
