Job Purpose & Job Summary
To provide a vital link between patients and healthcare professionals by delivering high-quality administrative and reception support alongside targeted care coordination activities. This role is designed to enhance patient experience and contribute to proactive care delivery across the PCN.
Reports to: PCN Non Clinical Director
Location: Based within the Primary Care Network (PCN) Hub and GP Practices on ad hoc basis as required
Hours: Part- time (22.5 hours per week)
Key Responsibilities:
- Greet patients in person and via telephone in a friendly and professional manner.
- Manage appointment bookings, cancellations, and queries using clinical systems.
- Handle incoming emails, and clinical correspondence.
- Maintain accurate patient records and uphold confidentiality at all times.
- Support basic data entry.
- Support patients to access appointments, services, and follow-up care.
- Help patients navigate health and social care systems by liaising with other professionals or agencies.
- Identify patients who may benefit from proactive contact (e.g., frailty, long-term conditions, recent discharge).
- Provide support to improve care continuity and reduce unplanned hospital admissions.
- Participate in MDT meetings or case reviews as appropriate.
- Support wider PCN initiatives in line with the Network Contract DES and other local Thurrock priorities as they emerge
Person Specification:
Essential:
- Previous experience in a GP practice, NHS, or care-related admin role.
- Excellent interpersonal and communication skills.
- Ability to prioritise workload and work both independently and as part of a team.
- IT literate with experience using patient record systems (SystmOne).
Desirable:
- Experience in care navigation, coordination or social prescribing.
- Knowledge of local health and care services.
- NVQ Level 2 or 3 in Business Administration, Customer Services, or Health & Social Care.
Due to the nature of this role, the successful candidate will be required to undergo an Enhanced DBS check and provide a full immunisation record as part of the pre-employment process.
Pay: £26,000.00 per year
