Job Title: Clinical Lead Reports to: Registered Manager / Responsible Individual
Location: Community – GAIN Care Ltd – Leeds Regulated Activity: Treatment of Disease, Disorder, or Injury (TDDI) and Personal Care Hours:
40 hours Full Time or Part time Job Purpose To provide strong clinical leadership, governance, and oversight of all clinical activities delivered under the regulated activity Treatment of Disease, Disorder, or Injury (TDDI).
The Clinical Lead ensures safe, effective, evidence-based care for service users within their own homes, in line with CQC Fundamental Standards, organisational policies, and professional codes of practice.
Key Responsibilities
1. Clinical Governance and Compliance Ensure all clinical care and interventions comply with CQC regulations, Health and Social Care Act 2008, and NMC/HCPC standards.
Develop, implement, and review clinical policies, procedures, and standard operating frameworks (SOPs).
Lead on clinical audits, service evaluations, and quality improvement initiatives. Monitor and maintain compliance with infection prevention and control (IPC) requirements. Prepare and present evidence for CQC inspections and support the Registered Manager with any improvement actions.
2. Clinical Oversight and Quality Assurance Provide oversight for all clinical aspects of care planning, delivery, and review.
Conduct clinical risk assessments and ensure individualised care plans reflect medical needs and risk management strategies.
Oversee medication management, including storage, administration, documentation, and incident review. Investigate clinical incidents, complaints, and safeguarding concerns, ensuring lessons learned are embedded in practice.
Ensure clinical audits and key performance indicators (KPIs) are completed and reported accurately.
3. Leadership, Supervision, and Staff Development
Provide clinical leadership, mentorship, and supervision to nurses, senior carers, and care staff. Assess and maintain staff clinical competencies, including medication administration, PEG feeding, wound care, epilepsy management, and other complex interventions.
Support the Registered Manager in delivering a robust training and development plan for clinical and care teams.
Promote a culture of learning, reflection, and continuous improvement across the service.
4.Risk Management and Clinical Safety Lead on clinical risk identification and mitigation, ensuring effective escalation processes are in place. Oversee emergency protocols for deteriorating conditions, seizures, anaphylaxis, or other urgent health situations.
Ensure safe delegation and accountability within clinical practice. Review and approve care plans for complex clinical interventions under the TDDI regulated activity.
5. Collaboration and Liaison Work collaboratively with GPs, community nurses, hospitals, and multidisciplinary teams to ensure continuity and safety of care. Support clinical assessments for new referrals, ensuring appropriateness for TDDI care delivery. Communicate effectively with service users, families, and healthcare professionals. Represent the organisation at clinical governance meetings, commissioning reviews, and external forums.
6. Documentation and Record Keeping Ensure all clinical documentation is accurate, comprehensive, and compliant with GDPR and professional standards. Lead on clinical audits of record-keeping systems, ensuring data integrity and confidentiality. Maintain clinical reports and contribute to the organisation’s quality assurance framework.
7. Safeguarding and Duty of Candour Act as or support the Safeguarding Lead for clinical matters. Promote and uphold the Duty of Candour, ensuring openness, honesty, and accountability in clinical practice. Ensure staff are trained and competent in recognising and responding to safeguarding concerns.
Qualifications and Experience Essential: Registered Nurse (NMC) or equivalent registered health professional (e.g., HCPC Allied Health Professional). Minimum of 3 years post-registration experience, including experience in community or domiciliary care settings. Proven track record of clinical leadership and quality improvement. Sound understanding of CQC regulations, clinical governance, and risk management frameworks. Excellent communication, leadership, and organisational skills.
Pay: £35,678.89-£41,335.30 per year
