Care Coordinator
Salary: £32 000 + Bonus
Location: Right at Home Tyneside
Hours: Monday to Friday, 8:00am – 4:30pm (45-minute lunch)
Start Date: Immediate (following notice period)

As our service continues to grow, we are looking for an experienced and highly organised Care Coordinator to join our established office team. Working alongside the Registered Manager and office team, you will coordinate approximately 3000 hours of care each week, supporting a team of 85+ Care Professionals while ensuring the highest standards of care delivery, compliance and customer service.
The Role
As Care Coordinator, you will play a vital role in the day-to-day management of our service, ensuring clients receive safe, effective and person-centred care while supporting and developing our Care Professionals.
Key Responsibilities
- Work closely with the Registered Manager to ensure the smooth day-to-day operation of the service.
- Produce efficient weekly rotas covering approximately 3000 care hours per week.
- Ensure continuity of care while minimising travel time and maximising carer satisfaction and efficiency.
- Follow the company's Care Coordinator Golden Rules to maintain operational excellence.
- Respond promptly and professionally to telephone enquiries from clients, families and staff.
- Manage daily changes to client and staff schedules, communicating updates quickly and accurately.
- Maintain accurate electronic records and ensure all care management systems remain up to date.
- Support compliance with CQC regulations and contribute towards achieving and maintaining an Outstanding rating.
- Supervise and support a team of Senior Carers.
- Complete staff supervisions, observations and annual appraisals.
- Prepare and review person-centred care plans for new and existing clients.
- Monitor individual and team KPIs and contribute to continuous service improvement.
- Work closely with the Recruitment Coordinator to identify staffing requirements.
- Arrange shadow shifts, competency assessments and ongoing staff development.
- Liaise with healthcare professionals and external agencies to ensure coordinated care delivery.
- Participate in a shared out-of-hours on-call rota.
What We're Looking For
Essential
- NVQ Level 2 or 3 in Health & Social Care.
- Minimum of two years' experience within domiciliary care.
- Previous experience coordinating approximately 2,000 hours of care per week.
- Experience supporting or managing a team of approximately 60+ Care Professionals.
- Excellent understanding of CQC regulations and inspection standards.
- Strong organisational, planning and prioritisation skills.
- Excellent communication and relationship-building abilities.
- Ability to remain calm and professional under pressure and deliver solutions not problems.
- High attention to detail.
- Good IT skills, including Microsoft Office.
- Driving Licence with access to car
Desirable
- Previous experience using Access People Planner (or similar electronic care planning and rostering systems).
- Experience working towards Outstanding CQC standards.
- Previous supervisory or leadership experience.
