Lead Nurse / Lead Advanced Nurse Practitioner (ANP)

At IC24, we’re looking for a Lead Nurse / Lead Advanced Nurse Practitioner to join our Primary Care team at Herstmonceux Integrative Health Centre.
This is a senior clinical and leadership role where you’ll combine advanced clinical practice with responsibility for supporting, mentoring and developing the nursing and healthcare assistant team. You’ll help maintain high standards of care across the practice while influencing service delivery and clinical pathways.

You’ll be part of a not-for-profit organisation delivering care to over six million patients across the South and East of England, working to improve access, reduce pressure on secondary care services and ensure patients receive safe, timely and effective care.
What you’ll focus on
- Delivering advanced clinical care
You’ll provide assessment, diagnosis, treatment and management for patients presenting with undifferentiated and long-term conditions, delivering safe, evidence-based care in line with best practice and clinical guidelines.
- Leadership and team development
You’ll provide day-to-day clinical leadership for practice nurses and HCAs, supporting performance, development, supervision and appraisal processes, and fostering a culture of learning and accountability.
- Prescribing and clinical decision-making
As an Independent Prescriber, you’ll manage patient treatment plans following clinical assessment, ensuring safe, appropriate and effective prescribing within your scope of practice.
- Managing chronic disease and prevention
You’ll lead and support clinics for long-term conditions such as respiratory disease, diabetes and cardiovascular health, contributing to improved outcomes through proactive care and health promotion.
- Quality, governance and service improvement
You’ll work within IC24’s clinical governance framework, ensuring compliance with CQC, infection prevention and control standards, and contributing to audits, QOF performance and continuous improvement activity.
- Supporting training and workforce development
You’ll work closely with colleagues and the Learning & Experience team to support training, competency development, and career progression across the nursing and HCA workforce.
- Collaborative working across primary care
You’ll work closely with GPs, practice managers and wider clinical colleagues to identify population health needs, improve patient pathways and strengthen integrated care delivery.
What matters in this role
We’re not looking for one type of clinician - but there are some things that matter.
You’re likely someone who:
- Is a registered healthcare professional with current NMC, HCPC or equivalent registration
- Holds a recognised Advanced Nursing Practice qualification or equivalent advanced training
- Is a qualified Independent Prescriber
- Has experience working in primary care or general practice
- Has experience managing long-term conditions and chronic disease clinics
- Demonstrates strong leadership skills with experience supervising or mentoring staff
- Has excellent clinical assessment, diagnostic and decision-making skills
- Is confident working autonomously within professional boundaries
- Has strong communication and interpersonal skills
- Is committed to clinical governance, safeguarding and continuous professional development
