Job Overview
We are seeking a dedicated and experienced Care Team Leader to oversee and coordinate care services within community home care. The successful candidate will play a vital role in working alongside the Registered Care Manager leading a team of care staff, ensuring the delivery of high-quality personalised care, and maintaining compliance with relevant standards. Duties
- Lead and supervise a team of care assistants, ensuring the delivery of compassionate and professional care at all times
- Develop, review, and implement personalised care plans in collaboration with residents and their families
- Monitor the quality of care provided, ensuring compliance with regulatory standards and organisational policies
- Provide guidance and support to team members, fostering a positive and motivated working environment
- Assist with the organisation of staff rotas, ensuring adequate coverage for shifts including weekends and evenings as required
- Maintain accurate documentation related to resident care, staff records, and incident reports using IT systems
- Conduct regular team meetings to communicate updates, training requirements, and address any concerns or issues
- Support residents with daily activities, promoting independence whilst respecting individual preferences and needs
- Liaise effectively with healthcare professionals, families, and external agencies to coordinate resident care
Requirements
- Proven supervising experience within a care home or senior care setting
- Strong leadership skills with the ability to motivate and guide a diverse team
- Excellent communication skills for liaising with residents, families, staff, and external professionals
- Good organisational skills with experience in managing care plans and documentation using IT systems
- Valid driving licence is desirable for transporting residents or attending external meetings
- Knowledge of current regulations relating to adult social care and senior care services
- Ability to work flexibly across various shifts including evenings and weekends if necessary
- Compassionate approach with a genuine interest in providing high standards of person-centred care
- Preferable qualification SVQ 3 minimum
- Preferable Drivers license although not essential
Pay: £28,000.00-£30,000.00 per year
