Job summary
The Advanced Clinical Practitioner (ACP) will provide highly skilled, autonomous clinical care within Buckland Medical Practice, working across the four pillars of Advanced Clinical Practice: Clinical Practice, Leadership and Management, Education, and Research.
The post holder will undertake advanced assessment, diagnosis, treatment and management of patients presenting with both undifferentiated and diagnosed conditions. They will demonstrate expert clinical decision-making, provide evidence-based care, and contribute to the continuous improvement of services across the practice.
The ACP will work within their professional scope of practice and regulatory framework, providing holistic care for patients while supporting the wider multidisciplinary team.
Key responsibilities
Clinical practice
The ACP will:
- Undertake comprehensive history taking, physical examination and clinical assessment of patients presenting with acute, chronic and complex conditions.
- Assess, diagnose and manage patients presenting with undifferentiated and undiagnosed health concerns.
- Develop and implement evidence-based treatment and management plans.
- Request, interpret and act upon diagnostic investigations including pathology and imaging results.
· Practise as an Independent Prescriber (where qualified), undertaking assessment, diagnosis, prescribing, monitoring and review of medications within professional scope and national guidance.
· Undertake medicines optimisation and structured medication reviews where appropriate.
- Manage patients with long-term conditions and complex co-morbidities.
- Provide same-day urgent care and clinical triage services.
- Recognise and respond appropriately to emergency presentations.
- Refer patients to secondary care, community services and other healthcare professionals as appropriate.
- Undertake home visits where required.
- Promote health education, prevention and self-management.
- Participate in vaccination and immunisation programmes.
- Maintain accurate, contemporaneous and legally compliant clinical records.
- Work collaboratively within the multidisciplinary team to ensure seamless patient care.
- · Provide assessment and treatment of same-day acute presentations.
- · Undertake telephone, video and face-to-face consultations.
- · Manage minor illness and minor injury presentations.
- · Contribute to long-term condition management clinics as required.
- · Participate in home visiting services when clinically appropriate.
- · Support achievement of QoF, IIF and other contractual requirements.
- · Undertake home visits for housebound, frail, vulnerable or acutely unwell patients who are unable to attend the practice, carrying out comprehensive assessment, diagnosis, treatment, care planning and referral as appropriate.
- · Assess and manage patients with frailty, falls risk and complex health needs within their own homes, working collaboratively with GPs, community services, care homes and other multidisciplinary team members to ensure continuity of care.
- · Support end-of-life and palliative care reviews through home visits where clinically appropriate.

- Support workforce planning and service delivery improvements.

- Assist in monitoring clinical performance indicators and outcomes.
- Participate in risk management, significant event reviews and learning from incidents.
- Support the implementation of local and national healthcare priorities.
Learning and development
The ACP will:
- Support the learning and development of colleagues across the practice.
- Act as a mentor, assessor or supervisor where appropriately qualified.
- Facilitate teaching and clinical education sessions.

The ACP will:

Qualifications
· Registered healthcare professional with:
- Nursing and Midwifery Council (NMC); or
- Health and Care Professions Council (HCPC); or
- General Pharmaceutical Council (GPhC).
· Master's Degree in Advanced Clinical Practice; or
· Equivalent advanced practice qualification with substantial evidence of ACP-level practice; or
· Currently undertaking an MSc Advanced Clinical Practice programme.
· Advanced Clinical Practice qualification recognised by the relevant professional body.
· Independent Prescribing Qualification (essential for professions where prescribing rights are available).
· Evidence of ongoing Continuing Professional Development (CPD).
· Teaching, mentorship or supervision qualification.
· Leadership or management qualification.
· Additional postgraduate clinical qualifications relevant to primary care.

Pay: £58,500.00-£68,250.00 per year
