Rainbow Care Solutions is a long‑established, CQC‑registered home care provider with over 20 years’ experience delivering safe, person‑centred support. We’re expanding our Discharge to Assess (D2A) / Reablement service in Hereford and are looking for an Assessment Coordinator who understands reablement: doing with, not doing for.
This is a key role supporting people to return home safely after hospital, regain independence, and reduce ongoing care needs through clear goals, timely reviews, and strong MDT working.
About the role
As our Assessment Coordinator for D2A/Reablement in Herefordshire, you will:
Complete initial assessments following referral/discharge, capturing strengths, risks, and what matters to the person
Develop and review reablement-focused care plans with clear outcomes and step-down approaches
Carry out risk assessments and ensure records are accurate, up to date, and CQC-compliant
Support timely reviews (including weekly reviews where required), responding quickly to changes in need
Work closely with therapists/MDT partners to support mobility, daily living skills, and safe routines at home
Carry out spot checks to maintain quality and best practice in the field
Provide supervisions, support and feedback to care staff delivering reablement
Escalate and support management of incidents, concerns and complaints in line with policy
Participate in on-call as required to ensure continuity and safety
About you
We’re looking for someone who:
Has experience in domiciliary care, reablement, hospital discharge, or a senior care role (Senior Carer/Team Leader/Field Care Supervisor or similar)
Understands CQC standards, safeguarding, and person‑centred practice
Is confident completing assessments, care plans, reviews and risk assessments
Can communicate clearly with clients, families, staff and professionals
Is organised, calm under pressure, and able to work independently across Hereford and surrounding areas
Is confident using digital care systems for planning and record-keeping
Holds a full UK driving licence and has access to a vehicle
Has the right to live and work in England
