Trent Care Network Ltd is expanding its Community Health Team and is seeking a committed and patient-focused Healthcare Support Worker to support proactive, planned care across our member practices, care homes and housebound population.
This is an excellent opportunity to join a forward-thinking PCN delivering neighbourhood-based, multidisciplinary care across primary and community settings.
Role Overview
The Healthcare Support Worker will work as part of our PCN Community Team delivering compassionate support and improving the quality of life for others, for our housebound patients and care homes. You will support non-urgent clinical care including blood sampling, immunisation programme, clinical observations, long-term condition review and patient recall programmes.
You will work closely with our clinical team, care coordinators, social prescriber and other MDT colleagues to support patients with long-term conditions, housebound needs and care home requirements.
This is a mobile role requiring travel across the PCN footprint. Flexible working, no weekend or evening required.
Key Responsibilities
- Undertake non-urgent blood sampling (phlebotomy)
- Support Immunisation programme
- Carry out clinical observations (BP, pulse, oxygen saturations, BMI)
- Support long-term condition reviews
- Deliver care support to housebound and care home patients
- Record activity accurately in EMIS or SystmOne
- Participate in MDT team delivery
- The ability to work independently as well as in a team environment
- Escalate clinical concerns appropriately
Essential Requirements
- Experience in a healthcare support or care delivery role
- Phlebotomy qualification
- Strong communication skills with the ability to engage empathetically
- Basic clinical observation skills
- Ability to work within a multidisciplinary team
- Ability to travel across multiple sites
- Full UK driving licence and access to a vehicle
Desirable
- Primary care or GP practice experience
- Experience supporting long-term condition reviews
- EMIS or SystmOne experience
- Community care experience
- PCN or MDT working experience
Governance & Support
You will work under PCN clinical supervision and operational line management, with structured support and clinical oversight in place.
This role is aligned to the Additional Roles Reimbursement Scheme (ARRS) Healthcare Support Worker role definition.
