An exciting opportunity has arisen to join the Kirklees Personalised Care team.
Personalised Care gives people the same choice and control over their mental and physical health that they have come to expect in every other aspect of their life. Personalised Care is a key element of the NHS long term plan. It helps a range of people, from those with long term illness and complex needs through to people managing mental health issues or struggling with social issues which affect their health and wellbeing. It helps them make decisions about managing their health so they can live the life they want to live based on what matters to them, working alongside clinical information from the professionals who support them.
You will be working within the Personalised Care Team for the Mast Primary Network (PCN). The Mast PCN has five member practices covering areas including Scissett, Denby Dale, Skelmanthorpe, Shepley, Lepton and Kirkheaton and Kirkburton who work together to improve the healthcare of the local population together with local partners within the NHS, local authority and voluntary sector.
Health and wellbeing coaches (HWBCs) use health coaching skills to support people to develop the knowledge, skills, and confidence to become active participants in their own health so that they can reach their health and wellbeing goals. They may also provide access to self-management education, peer support and social prescribing.
Health and Well-being coaches support people to self-identify existing issues and encourage proactive prevention of new and existing health conditions. This approach is based on using strong communication and negotiation skills and supports personal choice and self-management.
They will work alongside people to coach and motivate them through multiple sessions, supporting them to identify their needs, set goals, and help them to implement their personalised health and well-being plan.
As part of this role, you will be expected to work and travel across the MAST PCN footprint and therefore will be very difficult to do this role without access to transport.
Willingness to work flexible hours, including evenings and weekends on occasion.

This role is 37 hours over 5 days. Monday – Friday.
This is a fixed term post for 12 months.
