About the role:
Hillingdon Home Care Ltd is a CQC-registered domiciliary care agency based in Uxbridge, delivering compassionate, person-centred care to clients across the London Borough of Hillingdon. We are expanding our clinical services and are looking for a passionate and experienced Registered Nurse to join our growing team. This is a rewarding community nursing role where you will work with clients in their own homes — supporting them to live independently with dignity. You will lead on clinical care planning, work alongside our dedicated care team, and help shape our nursing care service as we grow.
What you will do:
- Conduct comprehensive nursing assessments for new and existing clients
- Develop, review and update individualised care plans - Administer and manage medications, including complex regimens
- Provide clinical interventions including wound care, catheter care, PEG feeding, and stoma management
- Monitor clients' health and escalate concerns to GPs and MDT teams
- Supervise and support care workers in the delivery of personal care
- Complete accurate clinical records in line with NMC standards
- Liaise with hospitals, GPs, district nurses and social workers
- Support hospital discharge packages for clients returning home
- Contribute to the development of clinical policies and procedures
What we are looking for:
Essential:
- Registered Nurse (RN) — valid NMC PIN required
- Minimum 2 years post-registration nursing experience
- Experience in community, district nursing, or home care settings
- Strong clinical assessment and care planning skills
- Excellent communication and interpersonal skills
- Compassionate, patient and person-centred approach
- Full UK driving licence and access to own vehicle
- Enhanced DBS (we will process this if not held)
- Right to work in the United Kingdom
Desirable:
- Experience with dementia, mental health or complex care clients
- Knowledge of NHS Continuing Healthcare (CHC) process
- Previous experience in a community or domiciliary care setting
- Nurse prescribing qualification
- Wound care or tissue viability training
