West Berkshire Council is seeking a dedicated Dementia Practitioner to join our Adult Social Care team, reporting to the Registered Manager at Birchwood Care Home.
About the Role
As a Dementia Practitioner, you will play a crucial role in delivering exceptional care and support to individuals with dementia. Your key responsibilities include:

Working collaboratively with the care team to ensure the highest standards of care are met
- Providing guidance and support to staff in the delivery of person-centred care
- Conducting assessments and reviews of individual needs
- Implementing appropriate care plans and monitoring progress
- Ensuring compliance with relevant legislation and regulations
- Building positive relationships with residents, their families, and external stakeholders
About You
The ideal candidate will possess the following skills and attributes:

- You will also be entitled to a generous annual leave entitlement starting at 27 days per annum (plus public holidays) Rising by an additional 5 days after 5 years’ service.
