We are looking for a Community Support Worker to help older people across the Great Yarmouth District remain independent and well at home.
In this role, you will work directly with older people and their families to understand their needs, provide practical and emotional support, and connect them with local services and community activities. You’ll carry out home visits, offer telephone and online support, and help individuals manage changing health or care needs.
Key responsibilities include:
- Completing person-centred assessments and support plans
- Carrying out basic risk assessments and wellbeing checks
- Providing information, advice and signposting to relevant services
- Acting as an advocate where needed
- Supporting delivery of wellbeing activities and workshops
- Keeping accurate and confidential records
We’re looking for someone who is compassionate, organised, and confident working independently in the community. Experience and knowledge in health, social care, or community work is a must, but just as important is a genuine commitment to supporting older people.

Additional Information:
Working hours for this role can be flexible based on the individual (but ideally between 18.5-30).
The role will require travel with agreed set location.
