This is a 3-month contract role with a local authority, designed to provide high-quality, person-centred case management services to adults with care and support needs, their carers, and families. The post holder will play a crucial role in promoting independence, wellbeing, and positive outcomes through strengths-based assessments, support planning, reviews, and care coordination.

Manage a caseload of adults with a range of care and support needs.

Undertake strengths-based assessments, reviews, and support planning in accordance with statutory responsibilities and best practice.

Identify individual strengths, assets, goals, and support networks to promote independence and wellbeing.

Monitor and review support arrangements to ensure services remain effective and meet changing needs.

Develop and implement person-centred support plans that maximise independence and choice.

Experience working with adults within social care, healthcare, supported living, community services, or the voluntary sector.

Experience managing caseloads and supporting individuals with varying levels of need.

Experience carrying out assessments, support planning, reviews, or coordinating services.

Level 3 Qualification in Health and Social Care or equivalent.

Alternatively, willingness to work towards achieving a Level 3 Health and Social Care qualification.

Working hours: 35 hours per week.

Location: 52d Drayton Park, London, Islington, N5 1NS, United Kingdom.

Enhanced DBS required.

Work pattern: Minimum of 3 days in the office and 2 days working from home.

Schedule: Bi-weekly.
