Summary
We are seeking a Care Coordinator to join our team in Pershore / Evesham. This vital role involves overseeing senior care programs and ensuring the delivery of exceptional support services. As a key member of our organization, you will contribute to our mission of providing excellent care and support.
Responsibilities
- Coordinate and manage senior care activities, ensuring compliance with care standards
- Develop and implement personalized care plans tailored to individual needs
- Supervise staff and volunteers, providing leadership and guidance
- Communicate effectively with clients, families, and healthcare professionals
- Oversee daily operations within the care program
- Ensure safety protocols are followed and quality standards are maintained
- Maintain accurate records and documentation related to care activities
- Support continuous improvement initiatives within the care services
- Qualifications
- Proven experience in supervising or managing senior care services
- Strong leadership and communication skills
- Valid driving license is preferred for transportation duties
- Ability to develop comprehensive care plans
- Excellent interpersonal skills for liaising with clients, families, and team members
- Knowledge of IT systems relevant to care management
- Previous experience in a home care setting is advantageous but not essential
- Commitment to delivering high-quality support and service
- Picking up care visits which cannot be covered due to sickness or other reasonable reasons
- Previous Care Co-ordinator experience
