Job Description - Care co-ordinator for Social Prescribing Team
Job Purpose:
The post holder is a Care Coordinator, who acts within appropriate boundaries, working primarily with the Social Prescribing Team within the Primary Care Team. The Social Prescribing Team work alongside GPs looking after patients’ non-clinical needs. We recognise that lots of thing affect wellbeing and not everything can be fixed by medicines. The Social Prescribing Team help patients find non-clinical support and solutions to help them feel better. The Care Coordinator provides vital support to the Social Prescribing Team by doing administration support, researching resources available across the city that patients can access and supporting a small number of individual patients.
The post holder will be an important part of the Social Prescribing team, building relationships with other community providers, community services and working closely in partnership with other staff within the PCN.
Job Description:
Work alongside Social Prescribers to help manage and assist aspects of their caseload and responsibilities.
Play an active role (calls to patients) in data gathering for the pro-active care initiative
Research new available resources using the internet, phone and face to face visits
Maintaining the resources database
Data entry to support reporting
Manipulating our unique database to provide reporting
Managing referral allocation daily and waiting list management
Organising group session administration and patient communication
Supporting a small number of patients to access support
Providing support to Portsmouth Social Prescriber Team by arranging meetings, minute taking, arranging speakers and training
Managing budgets and keeping accurate records
Designing newsletters for patients and internally
General admin support for the team which can include referrals, general communication, diary management and training
The role may involve innovative project work in line with new and future opportunities that are identified by Island City Network
Person Specification
Essential
High level of written and verbal communication skills and the ability to communicate effectively both verbally and in writing, with people, their families, carers and other stakeholders
Good working knowledge of Word, Excel, PowerPoint and Outlook
Ability to maintain effective working relationships with all colleagues and to work flexibly and enthusiastically within a team or working under own initiative
Ability to identify risk and assess and manage when working with individuals

Essential
GCSE in English and Maths
Ability to use Microsoft Office applications Word, Excel, PowerPoint, Outlook

Willing to travel across PCN sites if / when required
Desirable
Experience of working in a healthcare setting
Working knowledge of SystmOne
Ability to work collaboratively, liaising with other stakeholders as needed for the collective benefit of patients including GPs, Practice Nurses, healthcare professionals and other PCN staff
Understanding of the wider determinants of health, including social economic and environmental factors and their impact on communities, individuals, their families and carers
Ability to recognise and work within limits of competence and seek advice when needed
Familiar with local resources and services and how to access them.

Information Governance
The practice is registered under the Data Protection Act 1984. The post holder will comply with the Data Protection Act and the Access to Health Records Act, the General Data Protection Regulations 2018 and the practice policies regarding information governance at all times. You must not at any time use the personal data held by the practice for a purpose not described in the Register entry or disclose such data to a third party. If you are in any doubt regarding what you should and should not do in connection with the Data Protection Act, then you must contact your line manager
