JOB PURPOSE:
The Senior Care Coordinator will be responsible for coordinating the day-to-day delivery of safe, effective and person-centred home care services, ensuring clients receive high-quality care that meets their individual needs.
The post holder will support the Registered Manager in the operational management of the service, regulatory compliance, quality assurance, workforce management and continuous service improvement, while ensuring the smooth and efficient running of daily operations.
KEY DUTIES AND RESPONSIBILITIES:
1. Care Coordination and Service Delivery: Coordinate the day-to-day delivery of care services by planning staff rotas, allocating care visits and ensuring continuity of care. Monitor service delivery using electronic care management systems and respond promptly to changes in client needs and operational requirements.
2. Client Assessments and Care Planning: Undertake client assessments, complete risk assessments and develop person-centred care plans under the direction of the Registered Manager. Ensure care documentation remains accurate, compliant and regularly reviewed.
3. Client Onboarding: Coordinate the admission of new clients by completing assessments, preparing care documentation and ensuring all contractual paperwork is completed before care commences. Arrange introductory visits and support clients and families throughout the onboarding process.
4. Workforce Planning and Staff Coordination: Plan and maintain efficient staff rotas, allocate care visits and manage changes to staffing requirements. Provide day-to-day support to care staff and coordinate emergency cover to ensure uninterrupted service delivery.
5. Recruitment and Staff Development: Support the Registered Manager with recruitment, interviews, pre-employment checks, inductions and mandatory training. Assist with staff supervisions, competency assessments, probation reviews and annual appraisals.
6. Quality Assurance: Conduct spot checks, care audits and competency assessments to ensure care is delivered in accordance with care plans, company policies and CQC standards. Monitor service quality, analyse feedback and support continuous improvement initiatives.
7. Regulatory Compliance: Support the Registered Manager in maintaining compliance with CQC Regulations, Fundamental Standards and all relevant legislation. Assist in preparing for inspections, maintaining policies and implementing action plans arising from audits and inspections.
8. Safeguarding: Promote a strong safeguarding culture by recognising, reporting and escalating safeguarding concerns in accordance with organisational policies and statutory guidance. Support safeguarding investigations and maintain accurate records.
9. Medication Management: Monitor medication administration records (MAR Charts), identify medication concerns and support medication competency assessments. Promote safe medication practices and compliance with company procedures.
10. Health, Safety and Infection Prevention: Promote safe working practices by supporting workplace risk assessments, accident reporting and infection prevention and control procedures. Monitor compliance with health and safety policies and lone working arrangements.
11. Electronic Care Systems and Administration: Maintain accurate client and staff records using electronic care management systems. Produce operational reports and ensure documentation is completed accurately, securely and within required timescales.
12. Payroll and Timesheet Support: Verify staff working hours, travel time, mileage and timesheets prior to payroll processing. Support the Registered Manager in resolving payroll queries and maintaining accurate payroll records.
13. Client Invoicing Support: Assist with reconciling delivered care hours against scheduled visits and support the preparation of accurate client invoices. Investigate discrepancies and liaise with the Registered Manager where necessary.
14. Communication and Partnership Working: Develop effective working relationships with clients, families, healthcare professionals, Local Authorities, NHS organisations and other partner agencies. Act as a key point of contact for carers and clients regarding day-to-day service delivery.
15. Business Development and Service Improvement: Support the Registered Manager with service development, marketing initiatives, tender submissions and business growth opportunities. Promote high levels of client satisfaction and contribute to continuous service improvement.
16. Deputising for the Registered Manager: Provide operational leadership in the absence of the Registered Manager, ensuring continuity of service, effective decision-making and compliance with organisational policies and regulatory requirements.
17. On-Call Responsibilities: Participate in the out-of-hours on-call rota, responding to emergencies, staffing issues and urgent client concerns to ensure continuity of care.
18. Administration and Record Keeping: Maintain accurate client, staff, recruitment, training, audit and operational records in accordance with GDPR, CQC requirements and company policies. Ensure documentation is complete, secure and readily available for audit and inspection.
REQUIRED SKILLS:
Essential Qualifications
- Level 3 Diploma in Adult Care (or equivalent).
- Level 5 Diploma in Leadership for Health and Social Care (Adults) or willingness to work towards it.
- GCSE (or equivalent) in English and Mathematics.
Essential Experience
- Minimum of 2 years' experience within a CQC-regulated domiciliary or home care service.
- Experience coordinating care visits and managing electronic rostering systems.
- Experience undertaking client assessments, risk assessments and care planning.
- Experience supervising or supporting care staff.
- Experience using electronic care management systems such as CareLineLive.
- Experience supporting CQC compliance and quality assurance activities.

Pay: £38,000.00-£40,000.00 per year
