There is currently an opportunity within Blaenau Gwent for a passionate, committed and driven practitioner to progress their career to the position of Senior Social Work Practitioner.
The opportunity is within the Disability 0-25 Team who support child/ young people and their family where a child has been diagnosed long term (i.e. permanent) severe learning, physical or sensory disability and which causes them significant difficulty in their daily living. The team offers a high level of support to children/young person and their families by liaising with the relevant statutory and voluntary agencies to provide and monitor individual packages of care and service provision. This involves support being provided on a Care and Support Plan, Child Protection Registration, Children Looked After, Court cases, transition work and Court of Protection.
A senior practitioner in the team is responsible for providing a high-quality service in relation to all aspects of the referral, decision making, assessment and case management process.
This will include chairing complex meetings, the supervision of staff, holding more complex cases within the team. Deputising in the absence of the team manager and ensuring performance and data information is properly collated/ recorded on the WCCIS system and monitored in accordance with quarterly reporting requirements.
