Job Overview
We are seeking a compassionate and experienced Care Manager to lead and coordinate high-quality senior care within our care home environment. Responsibilities
- Develop, implement, and regularly review individualized care plans for residents, ensuring they meet each person's unique needs and preferences.
- Supervise and support caregiving staff, providing guidance on assisted living, dementia care, Alzheimer's care, medication administration, and other specialized services.
- Coordinate with healthcare professionals to facilitate medical appointments, treatments, and medication management in accordance with best practices.
- Monitor residents’ health status closely, promptly addressing any changes or concerns to maintain optimal well-being.
- Lead staff training initiatives on topics such as senior care techniques, dementia care strategies, medication protocols, and safety procedures.
- Foster a positive team environment by supervising staff performance, conducting evaluations, and encouraging professional development.
- Ensure compliance with all regulatory standards related to nursing home operations, assisted living facilities, and home care services.
Experience
- Proven experience working in assisted living facilities or nursing homes with a focus on senior care.
- Demonstrated leadership skills with supervising or managing caregiving teams in a care home setting.
- Strong background in nursing or healthcare support roles involving dementia care and Alzheimer’s care.
- Hands-on experience with developing and managing care plans tailored to individual residents’ needs.
- Knowledge of medication administration procedures within a regulated environment.
- Familiarity with regulations governing assisted living environments and home care services.
- Excellent communication skills to collaborate effectively with residents, families, healthcare providers, and team members.
