This is a 12-month fixed-term contract to provide maternity leave cover.
The role is full‑time at 38 hours per week. The starting salary is £12.81 per hour, with a review upon successful completion of your probation period.

Main duties of the job
The Care Co‑ordinator role is an important first step in each patient’s journey. You’ll play a key part in helping people access the care and support they need, offering a welcoming first point of contact and keeping everything running smoothly behind the scenes. This is a varied and rewarding role that combines patient‑facing work, call handling, and essential administrative tasks.
In this role, you will:
- Book appointments and support patient enquiries, offering clear explanations and helping patients feel confident about their next steps.
- Use good judgement and strong communication skills to ensure that patients who need urgent attention are seen promptly and in a calm, organised way.
- Manage incoming and outgoing calls, taking accurate messages, directing calls appropriately, and ensuring every patient feels listened to and supported.
- Keep patient records up to date by filing correspondence, reports, and results promptly so clinicians always have the information they need.
- Process referrals efficiently and accurately.
- Handle insurance reports and private work requests, including managing copy notes through paper requests and iGPR.
Alongside these tasks, you’ll also:
- Welcome, assist, and direct patients to the most appropriate service or healthcare professional in a courteous, efficient, and reassuring manner.
- Provide general support to the practice team, projecting a positive and friendly image to patients and visitors, whether in person or over the phone.
- Carry out a variety of administrative duties that help the practice run smoothly, including secretarial and clerical support for clinical staff and other team members.
- Facilitate effective communication between patients, the primary care team, secondary care, and other healthcare agencies, helping ensure everyone stays connected and informed.

Full Job Description
The duties and responsibilities of the Care Coordinator may include any or all of the tasks outlined below. These may be adapted over time by the management team in response to changing practice needs, workload pressures, and staffing levels. Please note, not all responsibilities will apply to every postholder. Tasks may vary depending on the requirements of the service, and individuals may be asked to carry out duties that are appropriate to their role, training, and experience. Support will be available from team leaders and clinicians to ensure staff are confident and well-equipped to carry out their responsibilities.
Job responsibilities:
- Ensure an effective and efficient service is provided to patients and any other visitors to the practice.
- Project a positive and friendly image to patients and other visitors, either in person or via the telephone.
- Deal with all general enquiries, explain procedures and make new and follow-up appointments.
- Assist patients with Rapid Health forms, ensuring all information has been captured with accuracy to prepare for clinical conversations.
- Assist patients with Care Navigation, which could include signposting to pharmacists for minor ailments/physio.
- Using your own judgment and communication skills, ensure that patients with no prior appointment but who need urgent consultation are seen in a logical and non-disruptive manner.
- Manage all enquiries in a compassionate & effective manner.
- Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, and ensure procedures are completed.
- Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.
- Provide general administrative support to the GP’s and other health staff.
- Ensure correspondence, reports, results etc. are filed promptly and in the correct records, ensuring that all recent correspondence is available when patients are seen.
- Processing and distributing emails and incoming/outgoing mail, paper form & electronic.
- Ensure letters and relevant correspondence are scanned into the patients record promptly and accurately.
- Filing of Lloyd George notes ensuring strict alphabetical order is adhered to, summarising, photocopying & scanning notes.
- Ensure Lloyd George records are kept in good repair, with all necessary information on the outside cover clearly visible.
- Completing patient recall systems.
- Typing letters, reports and associated documents accurately.
- Dealing with 2WW referrals and ensuring safety netting (recall) is in place.
- Processing routine & urgent NHS and private referrals.
- Advise patients of relevant charges for private (non-General Medical Services) services, accept payment and issue receipts for same.
- Dealing with insurance reports/requests for private work and copy notes through paper requests and iGPR.
- Processing tasks as allocated.
- Ensure that all new patients are registered onto the computer system promptly and accurately.
- Processing deductions within the specified timeframe.
- Read Code patient information into the practice clinical system, when needed with the aid of the senior/deputy Care-Coordinator or Duty Manager.
- Ensure Advice & Guidance responses have been received and action rejected referrals.
- Processing OOH/111 reports.
- Highlight SG concerns as per the Practice procedure.
- Being proactive in the identification of Carers.
- Provision of refreshments for staff and visitors as required and keeping the kitchen area clean and tidy.
- Maintain a clean, tidy and clutter-free working area at all times.
- Complete room stocks for clinical staff.
- Help patients access NHS app and online services.
- Manage patient caseloads such as QOF lists, childhood immunisations, learning disability, mental health review appointments and maintaining child and adult safeguarding lists.
- Manage waiting lists for GP/Nurse appointments and clinics for minor ops/medicals.
- Representing patients in MDTs and care home meetings.
- Arranging interpreters, chaperones for complex needs
- Support vulnerable patients where needed, e.g. contact carer to help with transport.
- Adding and managing adastra list, planning for crisis prevention.
- Undertake any other additional duties appropriate to the post as requested by the partners or the practice manager.
- Premises: Open up premises at the start of the day when first to arrive, de-activate alarm and make all necessary preparations to receive patients. When last to leave at the end of the day, ensure that the building is totally secured, internal lights are off and the alarm activated.

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately

Health & safety:
The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include:
- Using personal security systems within the workplace according to practice guidelines
- Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
- Making effective use of training to update knowledge and skills
- Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
- Actively reporting of health and safety hazards and infection hazards immediately when recognised
- Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holder’s role
- Undertaking periodic infection control training (minimum annually)
- Reporting potential risks identified
- Demonstrate due regard for safeguarding and promoting the welfare of children and adults at risk.
