ApproCare is seeking a Full Time Care Co-ordinator to join our busy fast paced office team based in Bluetown, Sheerness. Coordination experience preferred but not essential as full training will be given. Care experience is ESSENTIAL.
The successful applicant will be required to work 5 days per week (primarily office based) and every other Saturday and Sunday (community based - please note: weekend work for this position will only be as needed/minimal).

Requirements
- All candidates should have a minimum of 6 months care experience ideally as a Care Co-ordinator, Senior Carer or similar
- All candidates must already have certification of OR be willing to undertake RQF/NVQ/QCF Level 3 in Health & Social Care
- 40 hours average per week (with overtime opportunities available if desired).
- 5 weekdays (or 4 if desired by the applicant) mostly office based and may also include every other weekend community based to include AM visits and PM visits (only IF required). Office Hours will typically be 8:45am to 5:15pm
- Ability to cope with a busy and demanding role, involving constant telephone contact with Community Care workers and Clients to arrange shifts and plan rotas.
- Candidates MUST be a driver with their own vehicle
- Candidates MUST be willing and able to assist in covering care calls themselves and in cases of urgent need.

Salary: £26,936 basic pay + on call bonus + overtime opportunities. CIRCA: £29,213.60

Reportable to: Care Manager
Duties Include
- To maintain an accurate and up to date staff rota
- To ensure that Community Carers are selected to Clients specific needs and requirements
- To ensure Clients and their families are kept fully up to date regarding any adjustments to rotas and staffing
- Issue Clients their timetables weekly (as required)
- Issue Community Carers their rotas weekly
- To ensure that all Community Carers have the required documentation necessary before starting work, including the client's name, address, telephone number, detailed assessment of requirements and travel routes.
- To maintain and update Client and Community Carer databases with relevant information
- To accept new referrals from Health Professionals
- To carry out Assessments on Clients - new and existing (in conjunction with Field Care Supervisor)
- To conduct risk assessments and update these as required
- To conduct Client Reviews / Spot Checks / Compliance Visits (in conjunction with Field Care Supervisor)
- Liaise closely with the Senior Team
- To upload shifts/new referrals to rostering system and process/manage all Community Carer holidays request forms
- To participate in our quality assurance processes alongside all our Senior Team, to include internal audits and rota weekly quality checks.
- Covering shifts when necessary and to take part in the on call rota.
