This is an exciting opportunity to play a key role in the delivery of an innovative 12-month pilot programme supporting individuals with learning disabilities in Billericay to access Annual Health Checks and to take meaningful action following these through personalised Health Action Plans.

Your impact: You will support individuals to engage with their health and help them achieve practical improvements in their wellbeing, working hands-on in this community-based role.
About you:
- You have experience working with people with learning disabilities
- You have experience supporting individuals in a care, health or community setting
- You have an understanding of person-centred approaches
- You have strong communication skills
- You are able to build trusting relationships with individuals and families
- You are able to work independently and as part of a team
- You are compassionate, patient and have a supportive approach
