About the Role
Chosen Care Group is looking for an organised, proactive and experienced Care Coordinator to support the smooth day-to-day running of our Domiciliary care services in Ilford.
This is a great opportunity for someone with experience in care coordination, rota planning, staff support and care documentation who wants to develop their career within a professional, supportive and CQC-regulated care service.
You will work closely with the Registered Extra Care Manager, senior care staff, care workers, residents, families, housing teams and external professionals to ensure safe, person-centred and high-quality care is delivered at all times.
Key Responsibilities
As Care Coordinator, you will:
Prepare and manage staff rotas to ensure safe staffing levels.
Respond to sickness, absence, emergency cover and changes in care needs.
Support assessments, care plans, risk assessments and reviews.
Ensure care records, daily notes, MAR charts and documentation are accurate and up to date.
Monitor care delivery against agreed care plans and commissioned hours.
Support care staff with day-to-day guidance, communication and problem-solving.
Assist with spot checks, supervisions, competency checks and staff development.
Work closely with residents, families, GPs, social workers and other professionals.
Escalate safeguarding concerns, incidents or changes in residents’ needs promptly.
Support audits, compliance checks and CQC inspection readiness.
Promote dignity, independence, safety and positive outcomes for residents.
About You
The ideal candidate will have:
Previous experience as a Care Coordinator, Senior Care Worker, Field Care Supervisor or similar.
Experience working in domiciliary care, Extra Care, supported living or residential care.
Strong rota management and care coordination skills.
Good understanding of care planning, risk assessments and person-centred care.
Knowledge of CQC standards, safeguarding and medication processes.
Excellent communication and organisational skills.
Ability to work calmly under pressure and manage competing priorities.
Good IT skills and experience using care management software.
NVQ/QCF Level 3 in Health and Social Care, or willingness to work towards further qualifications.
