Part-Time – 22.5 hours per week

We work closely with GP practices and Primary Care Networks (PCNs) to transform care pathways and improve outcomes for some of the most complex patients in the system.
Our services span urgent care, home visiting, population health, digital transformation, and pioneering programmes such as Long Covid support and weight management initiatives.
The Role
We are looking for an experienced and motivated Advanced Nurse Practitioner (ANP) to join our Home Visiting Service, covering the Lewisham borough.
In this autonomous and rewarding role, you will:

Deliver high-quality, patient-centered care in to at least 8 housebound frailty patients per day, in their own homes

Independently assess, diagnose, and manage patients with complex and undifferentiated conditions

Develop and implement treatment plans, including prescribing where appropriate

Work collaboratively with multidisciplinary teams to ensure seamless patient care

Provide clinical leadership and support to colleagues

Contribute to service development, audits, and quality improvement initiatives
What We’re Looking For
Essential
-
Registered Nurse with full NMC registration
-
MSc in Advanced Clinical Practice (or equivalent)
-
Experience working in General Practice (minimum 1 year)
-
Strong clinical assessment, diagnostic, and decision-making skills
-
Ability to work autonomously and lead clinically

Experience with EMIS clinical system
-
Excellent communication and interpersonal skills

Independent Prescriber qualification
-
Car owner with business insurance cover and a full clean driving licence – this role requires you to drive to patient homes. Acute home visit parking permit will be provided
Desirable

Background in leadership, service development, or quality improvement

Experience working within community or integrated care settings
