As well as our inpatient unit at Woodside Avenue, we work within the wider community including our Outpatient and Wellbeing services; our support to patients in their own homes; our Community Specialist Palliative Care services; and our Patient and Family Support teams, providing emotional and practical support to patients, their families, and carers.
The Inpatient Services Team provide holistic, skilled, high-quality, and patient centred palliative care to all in-patients.
Role Purpose
To deliver a high standard of nursing care as a member of the Inpatient Unit Service, providing clinical guidance, direction, and support to the unit. Working with other members of the Inpatient Services Team to ensure delivery of high-quality evidence-based care and adherence to the CQC fundamental standards.
Key Responsibilities

Care provision – Plan, implement and evaluate holistic care provisions while acting as a resource of clinical knowledge for hospice staff and external agencies. Deliver the full care provision, including but not limited to; giving clear reports on patient progress and concerns to the multidisciplinary team while also facilitating others to do the same, using evidence-based practice approach to all aspects of care, undertaking the administration of medications safely and in accordance with the hospice policies and procedures. Wound care management including, assessment, planning and evaluation, providing emotional and wellbeing support to patients, significant others, and fellow team members along with clear and accurate communications delivered with compassion and tact, supporting family and significant others through difficult and stressful situations

Supervision – provide a supervisory role to all HCAs and students on each shift, actively supporting induction programmes, student placements and other staff training and acting as a mentor to others to support their learning

Patient records – providing clear and concise handover information to peers and colleagues at shift changeover, updating records as necessary and noting changes and significant moves in patient progress and / or concerns while also advocating for patients and other service users, ensuring their views are known to the team

Risk management and continuous improvement – including but not limited to; identifying and reporting risks, carrying out patient risk assessments and applying immediate remedial action as required, proactively seek improvements and make suggestions for improvements, carrying out changes to process and practice as directed for continual service enhancement

Decision making responsibility is at specialist level - providing expert advice and skills across the In-Patient Services, making recommendations for changes to manager and consulting manager on decisions which affect the team and / or wider organisation.

Key working relationships including but not limited to; Daily/ weekly - Palliative Care team, patients, relatives and close friends of patients, consultants, specialist service providers, volunteers, bank staff, care departments across the hospice and external agencies. Occasional - HR, Finance etc within the hospice
