As our services continue to grow, we are seeking an experienced and motivated Clinical Case Manager to join our team across the Southampton region.
About the Role
As a Clinical Case Manager, you will work closely with clients and their families to assess needs, coordinate rehabilitation programmes and develop tailored support packages that promote independence, wellbeing and meaningful outcomes.
You will take a lead role in identifying and sourcing appropriate services, coordinating multidisciplinary input and ensuring that rehabilitation goals remain focused on each client's individual needs and aspirations.
