Job Overview
We are seeking a dedicated and experienced Senior Community Care Worker to join our team. This vital role involves providing high-quality care and support to individuals within their homes and care settings, ensuring their wellbeing, independence, and dignity. Duties
- Deliver personalised care plans tailored to each individual's needs, promoting independence and wellbeing
- Assist clients with daily living activities such as bathing, dressing, medication management, and mobility support
- Supervise and mentor junior care staff, fostering a positive team environment and ensuring high standards of care
- Maintain accurate documentation of care provided, including progress notes and incident reports
- Coordinate with healthcare professionals, families, and other stakeholders to ensure comprehensive support
- Organise and participate in regular team meetings to review client progress and care strategies
- Drive between different client locations as required, ensuring punctuality and reliability in service delivery
- Uphold health and safety standards in accordance with organisational policies and procedures
- · Availability to support rota requirements, including cover and on-call duties where required.
Skills
- Proven experience in senior or supervisory roles within a care home or community setting specialising in senior care
- Strong leadership skills with the ability to supervise and motivate team members effectively
- Excellent communication skills for liaising with clients, families, healthcare professionals, and team members
- Competent IT skills for maintaining accurate records and updating care plans electronically
- Valid driving licence with the ability to drive safely between client locations
- Knowledge of current care practices, safeguarding procedures, and relevant legislation
- Ability to develop and implement personalised care plans that meet individual needs
- Compassionate demeanour with a professional attitude towards client confidentiality and dignity
