Adults First Point of Contact (FPOC) - Lead a New Front Door for Adult Social Care
We are looking for an exceptional Team Manager to lead our new Adults First Point of Contact (FPOC) service – a multi-disciplinary team at the heart of Adult Social Care’s front door.
This is a unique opportunity to shape and lead a newly created service focused on prevention, early intervention, strengths-based practice and timely assessment. Bringing together Social Workers, Occupational Therapists, Assessment & Review Officers, Community Connectors and Advanced Practice expertise, the FPOC team will provide a coordinated first response that ensures residents receive the right support at the right time.
As Team Manager, you will provide leadership, direction and oversight for the service, driving high-quality practice, effective triage and strong performance. You will work collaboratively with partners across health, housing, voluntary and community sectors to create seamless pathways that promote independence and prevent unnecessary escalation into long-term care.

- Has experience managing Adult Social Care services and multidisciplinary teams.
