We are seeking experienced, innovative and highly motivated pharmacists to join the Medicines Optimisation Team within NHS Leicester, Leicestershire and Rutland and NHS Northamptonshire Integrated Care Boards (LNR ICB). These roles offer an exciting opportunity to lead strategic medicines optimisation programmes that improve patient outcomes, reduce unwarranted variation, support quality improvement and deliver value across the Integrated Care System.

Advanced Pharmacist Population Health and Long-Term Conditions
Advanced Pharmacist Antimicrobial Resistance (AMR)
Advanced Pharmacist Formulary and Interface
Advanced Pharmacist Community Pharmacy and Integration

Advanced Pharmacist Population Health and Long-Term Conditions
Lead medicines optimisation programmes that improve outcomes for people with long-term conditions, frailty and health inequalities. Working with system partners, you will use population health approaches and data-driven insights to support pathway improvement and consistent, high-quality care.
Advanced Pharmacist Antimicrobial Resistance (AMR)
Lead antimicrobial stewardship and antimicrobial resistance programmes across the Integrated Care System. You will support improvements in antimicrobial prescribing, medicines governance, policy implementation and collaborative working to promote safe and effective antimicrobial use.
Advanced Pharmacist Formulary and Interface
Lead the development and implementation of formulary and interface medicines pathways across the system. You will support evidence-based decision-making, NICE implementation, shared care arrangements and the safe transfer of care between healthcare settings.
Advanced Pharmacist Community Pharmacy and Integration
Lead initiatives that strengthen the role of community pharmacy within integrated and neighbourhood-based care. You will support service development, improve access to medicines and urgent care services, and enhance collaboration across healthcare settings.

Working for our Integrated Care Boards will require you contributing to a shared purpose of improving outcomes for the communities we serve as part of a wider health and social care system. The organisation’s ethos is rooted in integrity, collaboration and accountability, with a strong focus on putting its patient population at the centre of everything it does and delivering services to improve patient outcomes, patient experience and added years of optimised health. As part of the newly formed cluster bringing together NHS Northamptonshire ICB and NHS Leicester, Leicestershire and Rutland ICB, staff are expected to work flexibly and collaboratively across organisational boundaries to support a joined-up approach to strategic commissioning and service improvement. There is an expectation of 40% on patch working , with office bases at County Hall, Glenfield, Leicester and Haylock House, Kettering. Staff can choose which office is their primary base for contractual purposes and travel claims.
This role will require a minimum of 2 days ‘on patch’ working in Leicestershire, Northamptonshire and Rutland in our 2 main offices or partner sites
