Our Intermediate Care Service is looking for a motivated and organised Administrator to join our friendly and supportive team. This is an exciting opportunity to work at the heart of patient flow across North Cumbria, supporting clinicians and partner organisations to coordinate safe, timely and effective discharge planning.
You'll be responsible for providing high-quality administrative support across the Intermediate Care Service and Transfer of Care Hub, maintaining patient information systems, processing referrals, coordinating meetings and ensuring that key information is available to clinical teams when they need it.
This role is ideal for someone who enjoys variety, has excellent organisational skills and takes pride in delivering a professional service that makes a real difference to patient care every day.
The successful candidate will provide high-quality administrative support to the Intermediate Care Service, helping to coordinate referrals and support the safe and timely discharge of patients from Intermediate Care beds. The role includes maintaining accurate patient information across electronic systems including Patient Vision, EMIS and STRATA, ensuring data is up to date and reflects patients' progress through the service.
The post holder will work closely with colleagues across health and social care, liaising with system partners to facilitate effective communication and coordination throughout the discharge process. They will also provide information and updates to patients and their families, ensuring discharge arrangements are communicated in a professional, timely and compassionate manner.
This role requires excellent organisational skills, attention to detail and the ability to manage multiple priorities while maintaining accurate records and supporting the efficient delivery of patient care.
