About the Role
The Community Care Coordinator is responsible for coordinating and scheduling care services, liaising with service users, families, and healthcare professionals, and ensuring the effective delivery of high-quality care. The post holder will also undertake direct care and support duties with service users as required, ensuring that care is delivered safely, compassionately, and in line with organisational policies and regulatory standards.
You will act as a key point of contact between clients, care staff, healthcare professionals, and families, helping to ensure smooth and effective delivery of care.
Key Responsibilities
- Coordinate and manage care packages for service users in the community
- Develop and update care plans in line with individual needs
- Schedule and allocate care staff effectively to meet service demands
- Liaise with healthcare professionals, social workers, and families
- Monitor service delivery and ensure high standards of care
- Respond to changes in care needs and adjust plans accordingly
- Maintain accurate records and documentation
- Support with onboarding and supervision of care staff
In addition to coordinating care services, the Community Care Coordinator will be expected to provide direct care and support to service users as required. This may include:
- Delivering person-centred care and support in accordance with individual care plans.
- Assisting service users with personal care, where appropriate.
- Supporting individuals with daily living activities, including meal preparation, medication prompts, and community access.
- Promoting independence, dignity, choice, and wellbeing.
- Monitoring and reporting changes in service users' physical, emotional, or social needs.
- Maintaining accurate records of care delivered and observations made during visits.
- Working flexibly to ensure continuity of care and service delivery during staff shortages, emergencies, or periods of increased demand.
- Building positive relationships with service users, families, and other professionals to support positive outcomes.
Requirements
- Previous experience in a care coordinator, scheduler, or similar role
- Strong organisational and time management skills
- Excellent communication and interpersonal abilities
- Ability to work under pressure in a fast-paced environment
- Knowledge of care standards and regulations
- NVQ Level 3 in Health & Social Care (or equivalent)
- Full UK Driving Licence with vehicle
