Job Title: Extra Miles (Hospital Discharge Service) Support Worker
Team / Department: Community Services Team
Salary: £13.50 per hour
Contract Type: Fixed Term Contract until 31.03.28 (at which point it may be extended)
Hours: Part Time (16 hours per week)
Reports to: Extra Miles Manager
Background
St Paul’s Centre is a local Christian charity based in the centre of Crewe that has been well-established since 1986. Inspired by its Christian heritage, the charity runs community projects that look to provide hope and practical help to local households.
Our hospital discharge services (known as Extra Miles) provides safe and supported discharge and settling in services to patients from Leighton and Macclesfield hospitals.
A seven-day wrap around service ensures that patients who are isolated are supported to return to their home after a period of hospitalisation. This includes transport home from the hospital, a safety check on property and a needs assessment that leads to providing patients with the support they need to settle back at home.
Within the support process patients are linked to wider services they may need. Working within a person-centred model the service seeks to ensure the best possible experience for patients that will also serve to reduce avoidable re-admission.
Main Responsibilities
Referrals
· Respond to and action referrals from CEC Community Connectors promptly, while working in close collaboration with the Extra Miles Service Lead.
· Undertake a Risk and Needs Assessment of each client as part of the support process and as the service is delivered.
Transport
· Ensures vehicles are checked for safety prior to use and as per the procedures of the service.
· Ensures clients are safely seated in the vehicle and safely supported to enter their premises.
Residence Checks
· Ensure the home environment is safe for the client to return to, checking for health and safety hazards and making sure there is electricity, heating and running water.
· Ensures the client is left with basic food supplies.
· Ensures clients are able to use their communication devices (phones etc).
· Provides follow up appointment if required to assess and deliver on needs.
Seven-Day Support and Exit
· If required, provide a 7-day support package including shopping/prescription collection, lifts to clinical appointments, organising the removal/changing around of furniture, changing/replacing soiled bedding, food parcels from St Paul’s food bank and hot meals from The Wishing Well. As required, links clients with wider services offering bespoke or social support to provide ongoing support beyond our 7-day support package.
Health and Safety
· Ensure all legal and H&S requirements are adhered to.
· Works according to the policies of St Paul’s Centre and works to and contributes to the procedural framework developed for the Extra Miles Services.
· Uses a person-centred approach throughout, obtaining consent for service delivery ensuring clients have choice and options that create independence and maintain client safety.
Administration and Liaison
· Immediately act on/relay any safety concerns to Cheshire East Community Connectors keeping Extra Miles Service Lead fully informed.
· Undertake the completion of electronic contemporaneous observational notes regarding the client’s environment and wellbeing, ensuring these are submitted to the Community Connectors and the Extra Miles Services Manager immediately upon leaving the client, or at the earliest possible opportunity.
· Undertakes to meet weekly with the Service Manager and maintains strong communication links as required and submit weekly data statistics.
· Liaise with and support other Extra Miles team members, when required.
Your duties may be modified from time to time to suit the needs of the charity.
Please note that applicants must hold a full UK driving licence and have full eligibility to work in the UK. The successful applicant will be subject DBS clearance. We are unable to sponsor or provide visas for this role.
