The Community Mental Health and Wellbeing Hub will sit alongside and compliment those provided by primary and secondary care ensuring that by working together anybody who needs a service sees the right person at the right time, through a single point of entry. The role of a Mental Health Advisor has been specifically designed to provide wellbeing support to Bexley residents accessing the Community Mental Health and Wellbeing Hub. You can encourage, motivate and connect with people who access the service, working alongside the Multi-Disciplinary Team (MDT), to provide recovery orientated, solution focused, holistic care.
Key responsibilities include:
· Conducting focused one-to-one conversations of need with Bexley residents in various community settings, face to face and if needed via telephone or video calls where appropriate.
· Preparing accurate documentation, including client summaries, recommendations, and correspondence to support care pathways and communication with partner agencies, including GPs.
· Supporting audits and service quality activities under the supervision of clinical staff.
· Participating in team meetings and acting as advocates for clients by raising safeguarding concerns, risks, and wellbeing issues with senior staff when required.
· Developing and maintaining effective partnerships with community and statutory organisations to facilitate appropriate signposting and holistic support.
· Supporting individuals to improve their mental and physical wellbeing through psychoeducation, self-management strategies, and access to available community resources.
· Providing flexible, recovery-focused support tailored to individual needs, preferences, and personal goals.
· Enabling individuals to make informed decisions regarding their care, treatment, and support pathways.
· Assisting in the development and delivery of groups, workshops, and brief psychosocial interventions that promote community-based recovery and social inclusion.
Mental Health Advisors undertake supportive conversations of need to identify an individual's goals, strengths, and support requirements. These conversations are not clinical assessments and are intended to facilitate access to appropriate support, signposting, and recovery-focused interventions. Mental health advisor are supervised and support by clinical staff to ensure safety, quality and guidance with care pathway planning.

Pay: £26,500.00-£30,096.00 per year
